健康成年人膈肌呼吸运动的动态MRI初步研究

摘要:探讨动态呼吸MR成像对研究膈肌运动的可行性并将所测量的膈肌动度与肺功能检查相比较。运用单次激发快速自旋回波(SSHTSE)序列,对35例健康志愿者(男性21名,女性14名)进行MR动态扫描,并在1~2个深慢呼吸周期内采集图像。利用电影回放的方法获得膈肌运动的最高点和最低点,进而测量膈肌的运动幅度。比较左右侧膈肌的呼吸动度,并与肺功能结果进行比较,并利用时间距离曲线描述膈肌的运动特点。结果 左、右侧膈肌动度的平均值为12.81mm和9.11mm。膈肌的动度与肺功能指标FVC、FEV1以及FEV1/FVC、MVV和SVC之间具有明显的相关性。结论 左右侧膈肌动度之间的差异有统计学意义,动态呼吸运动成像能够很好的评价膈肌运动,具有潜在的临床应用价值。

Preliminary Observations of healthy adults’ Diaphragms with dynamic breathing magnetic resonance imaging
Abstract:The aim of this study was to show the feasibility of the diaphragmatic movement with dynamic breathing magnetic resonance imaging (BMRI) and to compare the results with pulmonary function test (PFT). Methods: The study included 21 men and 14 women. BMRI with Single Shot Turbo Spin Echo (SSHTSE) sequence was obtained during slow and deep inspiration and expiration. After examination, over cine-loop display, the highest and lowest positions of the diaphragm were identified and the distance of excursion was measured. Differences in the diaphragmatic excursion between left and right were compared. We have also correlated BMRI results with PFT. Results: Average excursion between deepest and highest point of diaphragm were 12.81 and 9.11mm, respectively, in the right and left side in healthy group. Significant correlations were found between FVC,FEV1,FEV1/FVC,MVV,SVC and MR results. Conclusions: The study showed that there were significant differences in diaphragmatic excursion between left and right diaphragm in healthy subjects. BMRI would seem to be a potentially useful method for showing diaphragmatic excursion.
