动态磁共振扫描研究肺部孤立性肿块对膈肌运动的影响

摘要:利用MR快速成像序列观察肺功能正常的肺部孤立性肿块患者膈肌的运动情况,探讨不同直径、位置的肿块对于膈肌运动功能的影响。运用单次激发快速自旋回波(SSTSE)序列,同时加用敏感性编码(SENSE)技术,对21例肺部孤立性肿块患者进行MR动态扫描,在2~3个呼吸周期内,连续采集通过气管隆突的中央冠状层面和通过肺尖的中央矢状层面(包括左、右侧)的图像,描记不同组别的健、患侧膈肌运动位移的时间-距离曲线,并比较不同直径、位置的肿块对健、患侧膈肌运动幅度的差异。结果 直径小于3cm的肿块健、患侧膈肌在呼、吸两相运动幅度没有差异;直径大于5cm的肿块健、患侧膈肌在吸、呼两相运动幅度均有显著差异;而直径3~5cm肿块健、患侧膈肌仅在吸气相运动幅度有差异。在吸气相,两肺下野的肿块较上野使患侧膈肌的运动幅度明显减小,右肺肿块较左肺使患侧膈肌的运动幅度明显减小;呼气相未见明显差异。结论 不同直径、位置的肿块对膈肌运动功能有不同程度的影响,动态MR成像能够很好的评价膈肌运动,对肺癌患者术前肺功能评价和肿瘤的放射治疗均具有较高的应用价值。

Evaluation of diaphragm motion during the breathing
Abstract:The aim of this study was to investigate diaphragm motion during the whole breathing cycle using magnetic resonance imaging (MRI) in solitary lung tumor patients. And the influence of tumor diameter, location on diaphragm motion were examined. Methods: Breathing cycles of 21 patients with a solitary lung tumor were examined using a SSTSE sequence (40~50 imaging in 2~3 breathing cycle), and SENSE technique has been to apply. All the patients have normal result on spirometry. Dynamic MRI was acquired at a fixed mid-coronal plane through the trachea, at 2 fixed mid-sagittal planes through the middle of each hemithorax. Diaphragm motions were assessed by cineloop viewing of sequential MR imaging and using time-distance curves continuously documented the distance changes from deep inspiration to expiration. Motion of tumor-bearing and corresponding contralateral non-tumor bearing regions was compared. Results: Patients with a tumor >5 cm showed a significant difference between the non-tumor bearing and tumor bearing hemithorax in the craniocaudal (CC) directions. But tumors <3cm showed no significant difference. With a 3~5cm tumor, difference proved to be significant in deep inspiration. And in inspiration, the lower tumors has more reduction in tumor bearing hemithorax than upper tumors, right tumors has more reduction in tumor bearing hemithorax than left tumors. Conclusion: Dynamic MRI is a simple noninvasive method to evaluate diaphragm mobility, and is a potentially useful tool for monitoring respiratory mechanics. Our results indicated that some partly and weakly injury of lung would must been attention in lung tumor patients.
