颈动脉超声体模血管扩张(FMD)的测量比较研究

摘要:通过设计实验对超声体模的血管扩张功能(Flow-mediated Dilation,FMD)进行测量,对各类测量方法做出定性定量的分析和比较,从而得到较好的评估方法用于颈动脉粥样硬化临床上早期的诊断和治疗。以颈动脉超声体模的实际尺寸作为金标准,分别在超声机平台(Ultrasonography-based Measurement Method,UMM)和计算机平台(Computer-based Measurement Method,CMM),使用三种测量方法--①横截面斑块高度测量(Transverse Plaques Height Measurement,TPHM),②横截面斑块面积测量(Transverse Plaques Area Measurement,TPAM)和③纵截面斑块高度测量(Longitudinal Plaques Height Measurement,LPHM),分别获取斑块的高度进行比较和分析。结果表明在 TPAM方法中,CMM比UMM更为准确;而在TPHM和LPHM中,CMM比UMM更为准确。CMM与UMM的测量结果在统计学上存在显著性差异(P<0.05),但在数值上相差较小。总体来上讲,UMM和CMM测量结果都接近于实际值,CMM的精确性优于UMM 。

Comparison of measurement methods of carotid flow-mediated dilation based on ultrasound phantom
Abstract: The aim of this study was to acquire great evaluation method used to the early stage diagnosis and treatment of carotid atherosclerosis in clinic by designing measurement experiment based on ultrasound phantom for Flow-mediated Dilation (FMD).And qualitative and quantitative analyses and comparison were made for these methods. The actual heights of simulated plaques in carotid ultrasound-phantom acted as the golden standard for judgment. Plaques heights measurement on two individual platforms, respectively - Ultrasonography-based Measurement Method (UMM) and Computer-based Measurement Method (CMM) - for three respectively measurement methods (①Transverse Plaques Height Measurement (TPHM) ②Transverse Plaques Area Measurement (TPAM), ③Longitudinal Plaques Height Measurement(LPHM).From these data results, analyses and comparisons were conducted to evaluate these measurement methods of the plaques height. The measurement results between UMM and CMM were statistically significantly different (P<0.05), but there was only a small difference in numerical value. For TPAM, UMM had a better performance in terms of accuracy than CMM; but for TPHM and LPHM, the results were just the opposite. Generally speaking, the accuracy of CMM was better than UMM.TPHM is similar to LPHM, but TPAM had obvious difference between both TPHM and LPHM..
