原发免疫性血小板减少症患者外周血CD4+T细胞中CD40L表达的研究

摘要:CD40/CD40L 是一对重要的共刺激分子,参与多种自身免疫性疾病的发生。近年来研究发现CD40L 在一些自身免疫性疾病女性患者中高表达。因而本研究中我们检测CD40L 在原发免疫性血小板减少症(ITP)患者外周血CD4+T细胞中的表达,分析CD40L与性别的相关性并探讨其在ITP发病机制中的作用。采用ELISA方法检测44名活动期ITP患者、12 名缓解期患者及26 名健康对照组血浆中的sCD40L。结果显示活动期ITP 患者血浆sCD40L 浓度较缓解期患者(P<0.05)及正常对照组(P<0.05)显著增高,缓解期患者与正常对照组无显著差异(P>0.05)。在活动期患者中,血浆sCD40L水平与血小板数呈正相关关系(P<0.0001,r=0.7721), 而与性别无关。进一步采用流式细胞术及实时定量PCR的方法检测CD40L蛋白及mRNA在外周血CD4+ T淋巴细胞中的表达,结果显示活动期ITP患者中CD4+T细胞中CD40L+细胞的比例及mRNA的表达明显高于缓解期(P<0.05)及正常对照组(P<0.05);同时女性ITP 患者CD4+T细胞上CD40L蛋白及mRNA的表达明显高于正常女性对照(P<0.05),而男性患者与男性对照无差异。综上所述,该研究发现活动期ITP 患者外周血浆sCD40L及CD4+T细胞上CD40L的表达较缓解期及正常对照组明显升高,且与血小板数目相关,提示其参与ITP 的发生。另外,女性患者外周血CD4+T 细胞的CD40L蛋白及mRNA表达高于正常女性对照组,男性患者与男性对照无明显差异,提示CD40L在女性ITP中的表达升高可能与女性ITP患病率较高相关。

Expression of CD40L in CD4+ T Cells From Patients with Primary Immune Thrombocytopenia
Abstract:CD40/CD40 ligand (CD40/CD40L) was one of the important costimulatory molecules,and shown to play a significant role in the maintenance of autoimmunity. Nowadays, it was foundthat CD40L is over-expressed in CD4+ T cells of some female patients. Thus,in this study, weexplored the expression levels and roles of CD40L in CD4+ T cells from primary immunethrombocytopenia (ITP) patients. Plasma soluble CD40 ligand (sCD40L) levels were measured in44 active ITP patients, 12 patients in remission and 26 healthy controls by ELISA, and the resultshowed that active ITP patients had significantly elevated plasma sCD40L levels than those ofpatients in remission (p<0.05) and normal controls (P<0.05). In active ITP patients, the plasma?sCD40L levels showed significantly positive correlation with platelet counts (r=0.7721, p<0.0001). We further detected the CD40L protein on the surface of CD4+ T cells by flow cytometric analysis and CD40L mRNA expressions in CD4+ T cells by real-time PCR, and found both the CD40LmRNA and protein expression in CD4+ T cells were significantly elevated in active ITP than those of patients in remission (p<0.05) and normal controls (P<0.05). In addition, the CD40L mRNA and protein expression in CD4+ T cells from female ITP patients were higher than those of female healthy controls (P=0.037). In conclusion, the active ITP patients had significantly elevated serum sCD40L levels and CD40L expression on CD4+ T cells than those of patients in remission andnormal controls, which suggested that CD40L abnormal expressions were involved in ITP. Moreover, CD40L over-expression in CD4+ T cell of female patients but not males may be one of the important reasons for female susceptibility to ITP.????
