骨性Ⅲ类错颌牙弓宽度与垂直向的相关性研究

摘要:目的:本实验研究骨性Ⅲ类错颌牙弓宽度与垂直向的相关性,以及Ⅲ类错颌畸形牙弓宽度与垂直骨面型之间的相关性。方法:选取兰州大学口腔医院正畸科门诊骨性Ⅲ类错畸形患者56例(女性29人,男性27人),取得所有研究对象的原始石膏模型和X线头颅侧位片。根据下颌平面角(MP-FH,SN-MP)大小不同分成高、均、低角三组。测量每组上下颌尖牙、第一前磨牙、第一恒磨牙间牙弓及牙槽弓宽度,计算同一样本上下颌对应宽度之差。检验三组测量数据的正态性态及方差齐性,然后进行三组测量值单因素的ANOVA统计及各组测量结果与下颌平面角的Pearson相关性检验。结果:在本实验骨性Ⅲ类错颌畸形中,上下颌第一磨牙间牙槽弓宽度高角组大于均角组和低角组,具有统计学差异(P<0.05);其他测量项目三组间无显著性差异。Pearson相关性检验表明:骨性Ⅲ类错畸形上颌第一前磨牙间牙弓宽度与下颌平面角呈负相关;上下颌第一前磨区牙槽弓宽度差值与下颌平面角呈负相关;其余无显著性差异。结论:骨性Ⅲ类高角病例上、下牙槽弓宽度有发育过度的倾向,上牙弓中段宽度与垂直骨面型存在相关性。

The relationship between arch width and vertical dimension of skeletal Ⅲ malocclusion
Abstract:Objective: To analyze the relationship between arch width and vertical dimension of skeletal Ⅲ malocclusion. Methods: 56 adults with skeletal Ⅲ malocclusion (female 29, male 27) visited in the stomatology hospital of Lanzhou university were selected as the subjects. Making original plaster modal and lateral cephalometric radiography for each patient. According to the value of mandibular plane angle (MP-FH, SN-MP), this patients were divided into three groups (high-angle, average-angle and low-angle groups). Maxillary and mandible arch width and alveolar arch width between canines, first premolars and first molars were measured for each group. Calculating the differences between the maxillary and mandibular width on the same sample. To test data\'s normality and homogeneity of variance, then, statistic analysis was finished by one-way ANOVA and Pearson\'s rank correlation test. Results: In skeletal Ⅲ malocclusion, the width of the maxillary and mandibular molar alveolar arch in the high-angle group was larger than that in the average-angle group and the low-angle group, with significant difference (P<0.05). There was no significant difference of the rest measured value in the three groups. Pearson correlation test showed that the dental arch width of maxillary first premolar was negatively correlated with mandibular plane angle, maxillary and mandibular alveolar arch width difference in the first premolar was negatively correlated with the mandibular plane angle in skeletal Ⅲ malocclusion. There was no significant difference of the rest measured value in the three groups. Conclusion: High-angle patients with skeletal III malocclusion had a tendency of excessive development of maxillary and mandibular alveolar arch width. There was correlation between maxillary middle arch width and vertical facial type in skeletal III malocclusion.
