i-PRF用于再生性牙髓治疗的临床疗效评价

摘要:目的:研究和评价使用第二代注射型富血小板纤维蛋白(injectable- Platelet-Rich Fibrin, i-PRF)对牙髓坏死的年轻恒牙进行再生性牙髓治疗(Regenerative Endodontics Procedures, REPs)的临床疗效。方法:选取2016年9月至2017年9月因发育性、龋源性和外伤性导致牙根停止发育的年轻恒牙10颗。两名医师按照2016年6月美国牙体牙髓病医师协会(American Association of Endodontists, AAE)更新的再生性牙髓治疗(REPs)的操作流程对患牙进行治疗。所有病例均详细记录治疗前中后及复诊期间内患牙根尖周病变愈合情况、牙根再发育的程度和牙髓电活力结果。结果:在6-18个月的观察期内, 10颗患牙均表现为根尖周病变愈合的好转。有7颗患牙表现出不同程度的牙根根管壁增厚,牙根长度增加,根尖孔已闭合或逐渐闭合的结果。1颗患牙出现牙髓电活力阳性结果。结论:采用注射型富血小板纤维蛋白(i-PRF)作为根管内支架进行再生性牙髓治疗(REPs)能够取得良好的临床疗效。

Observation of the clinical outcomes of regenerative endodontics procedures of immature permanent necrotic teeth using injectable-platelet-rich fibrin
Abstract:Objective: The purpose of this study was to discuss the clinical and radiological outcome of using injectable-platelet-rich fibrin (i-PRF), a second-generation platelet concentrate, in the revascularization procedure of immature teeth with necrotic pulp. Methods: Select 10 immature permanent necrotic teeth from which roots ceased to develop caused by trauma or carious exposure and dental anatomic variations from September 2016 to September 2017. Two clinicians operate REPs followed the guideline of \'\'Considerations for Regenerative Procedures\'\' which created by the American Association of Endodontists (AAE) Regenerative Endodontics Committee,updated June , 2016. The follow-up should consist of clinical and radiographic examinations. All cases were recorded in detail, including resolution of periapObservation of the clinical outcomes of regenerative endodontics procedures of immature permanent necrotic teeth using injectable-platelet-rich fibrinical radiolucency,increased thickening of the canal walls, continued root maturation and positive response to pulp test. Results: During the observation period of 6-18 months, 10 teeth showed improvement in the healing of periapical lesions, 7 teeth showed different degrees of increased thickening of the canal walls, continued root maturation and apical closure. 1 tooth elicited a positive response to an electric pulp test.Conclusion: i-PRF acted as the successful scaffolds for the immature tooth with necrotic pulp and is potentially an ideal and efficiency scaffold for regenerative endodontic procedures.
