视网膜功能成像--无创测量糖尿病患者的视网膜血氧饱和度及血管直径变化

摘要:糖尿病是临床上最常见的严重慢性疾病之一,在世界范围内患病人数和影响区域持续增加。糖尿病及其并发症严重地损害了人类的生命健康和生活质量,也给全世界带来了巨大的社会负担和经济负担。视网膜微血管的病变是最早出现的糖尿病微血管并发症,能造成视网膜组织因缺氧而被破坏,并出现视力损伤。如果没有及时发现和治疗,更会引起血管阻塞、新生血管增生、出血甚至失明等严重后果。本研究利用视网膜功能测量仪,对眼底进行结构成像来直接观察体内丰富的循环系统,并对视网膜的血氧饱和度及血管直径进行无创、定量的功能性成像测量。通过对糖尿病患者和健康人群进行视网膜功能成像并定量比较分析,其实验结果表明,在糖尿病患者中,视网膜动脉血氧饱和度相比于健康人群有显著性的提高,视网膜动静脉血氧饱和度差值也有显著性地增高。此外,在糖尿病患者和健康人群中,视网膜动静脉的直径均存在差异,且静脉的直径总大于动脉的直径。研究结果表明通过视网膜功能成像可以检测糖尿病患者的视网膜氧代谢和灌注率等参数的临床变化,帮助医生更好地观察、评估糖尿病患者的视网膜微血管的病理改变,并具有对糖尿病患者进行眼科并发症的早期诊断和预后评估的应用潜力。

Retinal functional imaging--measuring the retinal oxygen saturation and vessel diameter changes in diabetes mellitus patients
Abstract:Diabetes is a serious, chronic disease and it is one of the most common clinical syndromes. The diabetes cases and affecting regions are increasing steadily worldwide. Diabetes and its complications harm the health and life quality of human beings severely, and they cause significant individual and social cost and burden. The dysfunction of retinal microvessels is one of the earliest diabetes complications on microcirculations. It will bring damage to the retinal tissue due to ischemia, and cause visual impairments. If not diagnosed and treated in time, it may progress into retinal vessel occlusion, neovascular proliferation, hemorrhage, and even the blindness by stages. The fundus is well-known to be an ideal window to observe the complex vasculature structure in vivo. In this study, using the retinal functional imaging device, not only the structural information of the retina is imaged, but also the functional information such as the retinal oxygen saturation and vessel diameter is imaged and measured non-invasively and quantitatively. By comparing the functional imaging results of diabetes patients and healthy controls, significantly elevated arterial oxygen saturation and arterial-venous oxygen saturation difference are found in diabetes patients. The result also shows that there are significant differences between the average diameters of arteries and veins both in diabetes patients and healthy controls. The result of this study demonstrate that using the retinal functional imaging method, it could measure the clinical changes of retinal oxygen metabolism as well as the perfusion rate in diabetes patients. Also it could help doctors to better observe and evaluate the pathological changes of the retinal microvessels in diabetes patients. In addition, the retinal functional imaging technique has the application potential for the early diagnosis and prognostic evaluation of ophthalmic complications associated with diabetes.
