不同垂直骨面型的青年男性骨性III类患者侧貌比较

摘要:目的:比较成年男性骨性III类不同垂直骨面型的软组织侧貌特征,为正畸及正颌外科提供诊治思路及参考数据。方法:选择90名未经治疗的成年男性骨性III类患者拍摄CBCT,依据下颌平面角的大小(MP-FH和MP-SN),分为高角组,均角组,低角组三组,每组30人,应用6种侧貌软组织分析法中14个测量项目对每组进行测量,各组测量指标进行统计学分析,比较三组间差异。结果:测量项目中有统计学差异是:Bs-H线、Z角、Pog-Sn⊥FH。分别属于Holdway分析法、Merrifield分析法和Epker分析法。高角组Bs-H线测量值最小,与均角组及低角组相比,高角组颏唇沟最浅;低角组Z角最大,与高角组及均角组均有较大差异,提示低角患者面容突度更大;Pog-Sn⊥FH 在低角组测量值最大,与高角组及均角组相比,低角组颏部突出程度更大。结论:成年男性骨性III类患者不同垂直骨面型软组织侧貌存在差异,应依据患者的具体面容特征制定出更符合个体的治疗方案,以达到更加良好的治疗效果。

Comparison of facial profile in skeletal class Ⅲ male youth of different vertical facial skeletal types.
Abstract:Objective:This research compares the soft tissue profile characteristics of different vertical facial types of skeletal class III in male adult,providing diagnostic and therapeutic ideas and reference data for the field of orthodontics and orthognathic surgery. Methods:90 untreated male adult patients with skeletal class III underwent CBCT. Each group consisted of 30 subjects. 14 items of six different soft tissue profile analysis were used to measure each group. The measurement indexes of each group were statistically analyzed. The discrepancy between the three groups were compared. Results:The measurement items that have statistical differences were: Bs-H line, Z angle, Pog-Sn FH. They are respectively belong to Holdway analysis, Merrifield analysis and Epker analysis. The measurement value of Bs-H line was the smallest in the high angle group. Compared with the mean angle group and the low angle group,mentolabial furrow of the high angle group is more flatter;the measurement value of Z angle was the largest in the low angle group,which was significantly statistical different from that in the high angle group and the mean angle group, suggesting that he low angle group\'s facial protrusion was greater;the measurement value of Pog-Sn⊥FH in the low angle group were the largest .Compared with the high angle group and the mean angle group,the chin protrusion was greater in the low angle group. Conclusion:There are discrepancy features in soft tissue profile of different vertical facial types in male adult patients with skeletal class III. It is necessary to provide a more individual treatment plan basing on the specific facial features of the patients so as to achieve better therapeutic effect
