微种植体支抗辅助上颌前牙整体内收的临床研究

摘要:目的:比较两种尺寸的不锈钢弓丝联合相应高度牵引钩以滑动法矫治上颌前突患者时上前牙的移动方式,为临床实现整体内收前牙提供参考和依据。方法:选取2016年6月-2018年2月兰州大学口腔医院正畸科就诊的上颌前突患者共22人,按随机对照原则分为两组,对照组G1采用摇椅弓滑动法内收前牙,实验组G2采用V形曲弓丝滑动法内收前牙。所有病例均选择微种植体加强支抗,拔除双侧上颌第一前磨牙的矫治方案。关闭拔牙间隙前(T0)和关闭拔牙间隙后(T1)拍摄头颅侧位定位片并制取上颌研究模型并进行相应指标的测量。本研究统计学分析采用 Mann-Whitney U检验和 Wilcoxon 秩检验。结果:两组患者侧貌均得到改善,A点后移(p<0.05)。对照组上切牙切端平均压低量大于实验组(p<0.05),对照组关闭拔牙间隙所用时间少于实验组(p<0.05),其余测量指标差异不具有统计学意义(p>0.05)。结论:微螺钉种植体支抗下摇椅形唇弓在内收过程中对前牙有一定的唇倾和压低作用,前牙内收时更接近整体移动。摇椅弓组关闭拔牙间隙速率较V形曲弓丝组快。

Clinical Study of Micro-screw Implant Anchorage Assisting En-masse Retraction of Maxillary Anterior Teeth
Abstract:Objective:To compare the movement patterns of the upper anterior teeth in patients with maxillary protrusion by sliding mechaincs with two different size stainless steel arch wires combined with anterior retraction hooks, so as to provide reference for clinical realization of en-masse retraction. Materials and Methods:22 patients with maxillary protrusions from the Department of Orthodontics, Lanzhou University Stomatology Hospital from June 2016 to February 2018 were divided into two groups according to the principle of randomized control. The control group G1 was treated with a rocking-chair archwire. The experimental group G2 adopted V-shaped curved archwire to retract the anterior teeth. In all cases, the micro-implants were used to strengthen the anchorage and the treatment of the bilateral maxillary first premolars was removed. The x-ray skull lateral projections and research moedels were taken before(T0) and after(T1) anterior teeth retraction. Statistical analysis of this study used the Mann-Whitney U test and the Wilcoxon rank test. Results: The facial profile of both groups was improved, and point A was moved backward (p<0.05). The intrusion of the upper incisors was higher in the control group than in the experimental group (p<0.05). The time taken to close the extraction space in the control group was less than that in the experimental group (p<0.05). The difference of the other measurement indexes was not statistically significant (p>0.05). Conclusions: The rocking-chair archwire has labial incline and intrusion effect on the anterior teeth during retraction, which makes anterior teeth movement close to en-masse movement. And the time taken to close the extraction space is shorter than V-shaped curved archwire.
