不同支抗下隐形矫治器推上颌磨牙远移的三维有限元分析

摘要:目的:建立完整的上颌牙列及隐形矫治器的三维有限元模型,研究不同支抗下无托槽隐形矫治器远移上颌磨牙时前牙区的位移情况。方法:选取个别正常合女性志愿者一名,对其颌面部进行CBCT扫描,通过Mimics、Geomagic Studio12.0及Pro/E5.0等软件建立上颌骨、上颌牙列、牙周膜及隐形矫治器的三维模型,AnsysWorkbench作为有限元分析工具,以磨牙远中移动0.2mm为位移载荷,模拟无托槽隐形矫治器在不同支抗设计下,分步移动磨牙(颌内支抗)、整体移动磨牙(颌内支抗)、整体移动磨牙配合Ⅱ类牵引、整体移动磨牙配合微螺钉种植体支抗时,上颌前牙区位移趋势。结果:加载组一分步移动磨牙,前牙区的唇向位移量最小,支抗丢失最少;加载组二整体移动磨牙,前牙区唇向位移量约为分步移动的一倍,支抗丢失严重,造成前牙区不必要的往复运动。整体移动配合II类牵引和种植体支抗对于前牙区支抗控制较好,且种植体支抗在矢状向和垂直向的控制效果均优于II类牵引。对于前牙区支抗消耗:加载组二>加载组三>加载组四>加载组一。磨牙远移距离较短者,II类牵引和种植体支抗对于前牙的控制无明显差异。结论:无托槽隐形矫治推磨牙远移的过程中,临床常用的几种支抗方式,均伴有前牙区的唇倾,支抗消耗,且中切牙的移动量最大,尖牙移动量最小。若患者在磨牙远移的同时需要压低上前牙,建议使用微螺钉种植体支抗,增强支抗的同时增加垂直向位移;若患者为前牙直立或舌倾,则考虑减弱上颌支抗的设计,使推磨牙远移的反作用力唇展前牙,改善唇倾度。

Three dimensional finite element analysis of maxillary molar distalization with different anchorage invisible orthodontic techniques
Abstract:Objective: To establish a three dimensional finite element model of maxillary dentition and invisible appliance, and to analyze the effect of different anchorage methods on anterior tooth area when maxillary molars distalization. Methods: A female volunteer was selected and the maxillofacial region was scanned by CBCT. The three-dimensional models of maxilla, maxillary dentition, periodontal ligament and invisible appliance were established by Mimics, Geomagic Studio12.0 and Pro/E5.0 software.ANSYS WORKBENCH was used as the finite element analysis tool, and the distal movement 0.2mm of molars was used as the displacement load.When the simulated invisible appliance moves the molars (intramaxillary Anchorage) step by step, the whole moving molars with type Ⅱ traction, and the whole moving molars with the micro-screw implant anchorage under different conditions, the whole moving molars are combined with the type Ⅱ traction, and the whole moving molars are combined with the micro-screw implant anchorage under the condition of different.
