下颌无牙颌六枚种植体不同位点及不同固定修复设计的三维有限元分析

摘要:目的:建立无牙下颌骨、种植体、全牙弓式及分段式修复体模型;分析无牙下颌六枚种植体不同固定修复设计中种植体植入位点的不同对种植体周围颌骨应力分布的影响;从生物力学的角度评估和解释无牙颌六枚种植体固定修复不同设计的临床效果。方法:临床获取成人个别正常颌的上下颌骨的CBCT三维数据,在计算机上使用Mimics、3-matic、GeomagicStudio、SolidWorks等软件完成无牙下颌骨、种植体和修复体三维模型的建立;通过ABAQUS有限元软件分析在咬合力作用时不同设计的种植体周围骨质的应力分布大小。结果:无牙下颌6枚种植体全牙弓式固定修复中应力分布峰值数据较小的种植位点设计7、5、1及7、4、2及7、4、1;二段式为7、5、1和7、4、1。三段式固定修复应力分布峰值数据较大,均超过了皮质骨可承受最大应力。结论:所有设计中,种植体颈部周围皮质骨为应力集中区。全牙弓式更有利于降低牙槽骨吸收的发生率。三段式修复的种植体颈部存在较大的应力集中,可有较大发生骨质吸收的风险。

Three-dimensional finite element analysis of different six-implants position and different implants supporting fixed prosthesis in edentulous mandibular
Abstract:Objective:To establish a model of edentulous mandible, implants, mandibular full-arch and segmental-arch prosthesis; and to analyze the stress distribution of different sites in the full-arch and segmental-arch fixed prosthesis of edentulous jaw supported by six implants. To evaluate and clarify the clinical effects of different designs supported by six implants from the perspective of biomechanics. Methods:Obtaining the three-dimensional data of the upper and lower jaws of adult normal jaws by CT scanning. Establishing the model of edentulous mandible、implants and prosthesis by using Mimics, 3-matic, GeomagicStudio, SolidWorks on the computer. Analyzing the stress distribution of the supporting tissues of different six-implants sites designs and different implants supporting fixed prosthesis in mandibular by using the ABAQUS software.Results:The implants sites designs of 7,5,1, and 7,4,2, and 7,4,1 in the edentulous mandible with full-arch fixed prosthesis supported by 6 implants has a lower peak stress; the two-segmental-arch prosthesis designs are 7,5,1, and 7,4,1; and the three-segmental-arch prosthesis designs have a large value of stress distribution which are beyond the maximum stress of cortical bone can withstand. Conclusions:The cortical bone around the top of the implant and the top of the implant were stress concentration areas. The full-arch type is apt to reduce the incidence of alveolar bone resorption. There is a large concentration of stress in the top of the implant of three-segmental-arch prosthesis designs , which may have a greater risk of bone resorption.
