超脉冲CO2点阵激光治疗痤疮凹陷性瘢痕的系统评价

摘要:目的:系统评价超脉冲CO2点阵激光治疗痤疮凹陷性性瘢痕的临床疗效和安全性,为该方案适用于痤疮凹陷性瘢痕的治疗提供有力的证据。方法:计算机检索CNKI、PubMed、Wan Fang Data、The Cochrane Library 、Web of Science、CBM和Medline数据库中公开发表的超脉冲CO2点阵激光治疗痤疮凹陷性瘢痕的随机对照试验(RCT)的文献,检索时限均为自建库至2019年10月31日。同时查阅所纳入研究的参考文献。由 2 位评价员按照一定的纳入、排除标准,进行文献的筛选、资料提取和质量评价,并采用 RevMan 5.3 软件进行Meta分析。结果:共纳入11个RCT,合计1426 (T/C=712/714) 例患者。Meta分析结果显示:(1)总体疗效:采用超脉冲CO2点阵激光治疗的试验组与对照组相比较,临床治疗有效率提高,两组之间疗效的差异具有统计学意义[RR=1.19, 95%CI(1.13,1.25), P<0.00001];(2)瘢痕改善情况:试验组的瘢痕改善情况优于对照组,两组之间差异具有统计学意义[MD=-4.77, 95%CI(-6.95,-2.59), P<0.001];(3)不良反应:试验组和对照组在总体不良反应发生率之间的差异无统计学意义[Z=1.22, P=0.22];(4)患者满意度:试验组的患者满意度高于对照组,差异具有统计学意义[Z=2.68, P=0.007]。结论:超脉冲CO2点阵激光治疗痤疮凹陷性瘢痕疗效显著、瘢痕改善情况好、患者满意度高,值得临床进一步推广和运用。

Efficacy and Safety of Ultrapulse Fractional Carbon Dioxide Laser Therapy for Atrophic Acne Scars: A Systematic Review
Abstract:To provide strong evidence for ultrapulse fractional carbon dioxide laser therapy for atrophic acne scars, we systematically evaluated the clinical efficacy and safety of this therapy. Methods: CNKI, PubMed, Wan Fang Data, The Cochrane Library, Web of Science, CBM and Medline were electronically searched from inception to 31st October 2019 to collect the randomized controlled trialEfficacy and Safety of Ultrapulse Fractional Carbon Dioxide Laser Therapy for Atrophic Acne Scars: A Systematic Reviews (RCTs). And the references of eligible studies were also manually screened. Two reviewers independently screened literatures according to the inclusion and exclusion criteria, extracted data and assessed the quality of included studies. Then, meta-analysis was conducted using RevMan 5.3 software. Results: A total of 11 RCTs involving 1426 (T/C=712/714) patients were included. The results of meta-analysis showed that: Compared with the control group, the experimental group with ultrapulse fractional carbon dioxide laser therapy had a higher effective rate and there was statistically significant difference between the two groups (RR=1.19, 95% CI (1.13,1.25), P<0.00001). It showed significance statistically that the experimental group had higher clinical improvement than the control group (MD=-4.77, 95% CI (-6.95, -2.59), P<0.001). No significant difference between the two groups was observed (Z=1.22, P=0.22). The experimental group improved patient satisfaction and the overall patients’ satisfaction was significantly different between the two groups (Z=2.68, P=0.007). Conclusion: The ultrapulse fractional carbon dioxide laser therapy has significant efficacy, higher improvement and better patient satisfaction in the treatment of atrophic acne scars. It is a promising treatment worth further promotion and application in clinical practice.
