EGFR外显子19缺失突变及其在恶性甲状腺肿瘤发生发展中的作用

摘要:目的:研究EGFR外显子19缺失突变(EGFR exon 19 deletion)在恶性甲状腺肿瘤中的作用及其分子调控机制。方法:构建稳定转染EGFR外显子19缺失突变的人正常甲状腺滤泡上皮细胞株(HTFEC-ex19 del);EGF处理,通过划痕、Transwell、软琼脂克隆集落形成实验、失巢凋亡实验检测细胞的恶性转化能力;转录组测序分析(RNA-seq),比较人正常甲状腺滤泡上皮细胞株(HTFEC)和具有EGFR外显子19缺失突变的细胞株(HTFEC-ex19 del)中显著差异表达的基因。结果:临床发现具有EGFR外显子19缺失突变的甲状腺癌患者对一代EGFR抑制剂(Icotinib)治疗有效,进一步对50例各种类型的(恶性)甲状腺癌患者的二代测序分析显示4%具有EGFR突变。实验结果表明,EGFR外显子19缺失突变能以EGF依赖性的方式转化人正常的甲状腺滤泡上皮细胞,使其具有侵袭特性。RNA-seq分析结果显示,EGFR外显子19缺失突变后LAMC2、TRIB3等基因表达显著上调,CTSB、FASN等基因表达显著下调。结论:EGFR外显子19缺失突变通过上下调一些与甲状腺功能密切相关的基因表达,如LAMC2、CTSB等,使人正常甲状腺滤泡上皮细胞发生恶性转化,从而在甲状腺癌发生发展中发挥重要作用,靶向EGFR外显子19缺失突变可成为临床治疗的有效选择。

EGFR exon 19 deletion reveals its role in thyroid tumorigenesis
Abstract:Objective:To investigate the effects and mechanisms of EGFR exon 19 deletion in malignantthyroid cancer. Methods: The malignant transformationwasexamined by wound-healing, transwell, soft agar colony formation and anoikis assay between HTFEC and generated stable transfection cell line (HTFEC-ex19 del). The significantchanges in mRNA levels of differentially expressed genes were observed by RNA-seq analysis. Results:In our study, a thyroid-derived lung cancer patient who harbored EGFR exon 19 deletion was responded to the first generation of EGFR-TKI(Icotinib).We subsequently found EGFRmutation in 4% of cases in 50 thyroid cancer patients. We showed that EGFR exon 19 deletion can transform human normal thyroid follicular epithelial cells in an EGF dependent manner. Data of RNA-Seq from generated two cell lines (HTFEC and HTFEC-ex19del) uncovered EGFR exon 19 deletion affects the transcription of genes such as LAMC2,TRIB3, CTSB andFASN, which leads to malignant transformation of normal follicular epithelial cells.Conclusion:EGFR exon 19 deletion transformshuman normal thyroid follicular epithelial cellsin an EGF dependent manner, viaregulating the expression of LAMC2 and CTSB, which play a critical role in the development of thyroid cancer, and inhibitors targeting EGFR can be an effective therapy ofmalignant thyroid cancer.
