SEM法比较两种印模技术对二矽酸锂全瓷髓腔固位冠精确度的影响

摘要:[目的] 采用SEM法,体外比较传统印模和数字化印模技术对二矽酸锂(IPS e.max CAD)髓腔固位冠精确度的影响。[材料与方法] 筛选40颗离体牙(第一磨牙)固定至石膏模型上,按照统一标准进行髓腔固位冠牙体预备,后随机分为A组和B组。A组进行传统印模(乙烯基聚硅氧烷印模材料),后灌注石膏模型,采用CEREC系统进行髓腔固位冠制作。B组直接采用CEREC系统口扫仪进行离体牙数字化印模,按照A组相同标准制作髓腔固位冠。所有制作完成的髓腔固位冠严格按照临床粘接程序分别与对应预备后的离体牙完成粘接,后沿着颊舌向将其切开。采用扫描电子显微镜(SEM)分别测量各个截面髓腔固位冠在边缘、髓腔轴壁中点两处间隙值,计算A组和B组在该两点的平均值,并采用F检验和t检验对所得数据进行统计学分析(α=0.05)。[结果] A组和B组平均间隙值分别为18.96±5.05 μm和12.89±2.21 μm (P<0.05)。A组冠边缘间隙值为15.53±3.3 μm,髓腔轴壁中点为22.40±3.20μm,而B组分别为10.73±3.63 μm和15.05±3.90 μm。[结论] 采用传统印模技术制作的髓腔固位冠精确度低于数字化印模(P<0.05),但两者的误差均在临床可接受的范围内(< 90 μm)。

Comparison of the effects of two impression techniques on the accuracy of lithium disilicate endocrown by SEM method
Abstract:[PURPOSE] To compare the effects of two impression techniques on endocrown (IPS e.max CAD) fabricated with CEREC system by SEM method. [MATERIALS AND METHODS] 40 extracted teeth (the first molar) were selected and fixed on the plaster model, and the endocrwons were prepared according to an uniform standards, and then randomly divided into group A and group B. Group A was given a conventional impression method (vinyl polysiloxane impression material), followed by a gypsum model, and the CEREC system was used to produce the endocrown. The CEREC system\'s intraoral scanner was used directly on the extracted teeth in Group B, and the endocrowns were produced according to the same standards of group A. All the prepared endocrowns were bonded with the corresponding prepared extracted teeth in strict accordance with the clinical bonding procedure, and then they were cut along the buccal and lingual directions. Scanning electron microscope (SEM) was used to measure the gap values of the endocrown of each section at crown edge and the midpoint of the axial wall of the pulp chamber. And then, the average value of Group A and Group B at these two points were calculated, and the F test and t test were also used to perform statistical analysis. [RESULTS] The average gap value of group A and group B was 18.96±5.05 μm and 12.89±2.21 μm (P<0.05), respectively. The gap value of crown margin in group A was 15.53±3.3 μm, and the midpoint of the axial wall of the pulp chamber was 22.40±3.20 μm, while those in group B were 10.73±3.63 μm and 15.05±3.90 μm, respectively. [CONCLUSION] The accuracy of the endocrown made by conventional impression technology is lower than that of digital impression (P<0.05), but the error of both is within the clinically acceptable range (<90 μm).
